Question No-5 Whether there is any provision for financial assistance to a person for medical treatment?

Answer-  Yes. The beneficiary may apply for financial assistance from Red-Cross to the Collector and Chairman Red-Cross and Chief Minister’s Relief Fund . If the disease is serious like Cancer, T.B.,  the affected person may apply for financial assistance from Chief Minister's Relief Fund (C.M.R.F.) in prescribed form through Collector.

Question No-1 Issue of Nationality certificate.

Ans. You can obtain a nationality certificate by submitting an application form No.1 of miscellaneous certificate (rule-4)

  • In duplicate.
  • Affix a court fee worth of Rs.3/-
  • Along with an affidavit .
  • File application before the A.D.M./Officer in Charge  G&M., Collectorate , Keonjhar.

Question No-2  Who is eligible to get the certificate ?

Ans. A person who is Indian by birth or acquired citizenship in Indian is eligible to get the certificate.

Question No-3  Who is competent authority to issue nationality certificate ?

Ans. The Collector and District Magistrate concerned is the competent authority to get the certificate.

Publication Of Magazine/News Papers

Question No-4  What is the Procedure for filing of declaration for publication of magazine/News-papers ?

          A person interested in starting a news paper/magazine (daily or periodical) is required to contact The District . Magistrate  or Sub-Divisional Magistrate in whose jurisdiction .The place of the proposed magazine / newspapers is situated and file a declaration before him in the prescribed form(Annexure-I).In the declaration, the publisher is to mention particulars of the proposed newspaper as are asked for.He is also required to give a list of alternative titles in the order of preference .

Question No-5 What is the duty of the Magistrate?

          The Magistrate before authenticating the declaration has to make an enquiry from the Register of Newspapers for India (at Shastribhawan, A wing, Dr. Rajendraprasad Road, New Delhi) whether the title(s) proposed aren't the same or similar to that of any other publication published in the same Language, throughout the country or in any other language in the same state. The proposed title(s) are checked against a catalogue of the existing titles maintained in the Register’s office. A reply is then sent to the Magistrate about the availability or other wise of the title. The Magistrate communicates the same to the intending publisher. If the title is available, and the magistrate so competent is to send the authenticated copy to R.N.I. New Delhi.

Solatium Scheme Fund

Question No-1 What is a Solatium scheme ?

Ans. It is a scheme of Social Welfare and justice to victims of Hit and Run Motor Accidents of Govt. of India by the 1989 Amendment of Motor vehicle Act. created the solarium Fund for payment of compensation to such victims.

Question No-2 Who are eligible to get compensation under this scheme?

Ans. If the death/injury to the victims is caused due to9 an motor accident and the Vehicle is not traced out, the cases are eligible to get compensation amount under this scheme.

Question No-3 What amount is to be given /sanction under this scheme ?

Ans. In case of death an amount of Rs.25,000/- is to be given/sanction in favor of the Legal heir of the deceased and in the case of serious injury, an amount of Rs. 12,500/- is to be given /sanction in favor of the injured person.

Question No-4 What is the procedure of sanction ?

Ans. On receipt of application from the claimant the Claim Enquiry Officer maintains a case record and after enquiry, the case record along with the claim enquiry report is submitted to the Collector-cum-Claims settlement commissioner for necessary sanction. Here , the Sub-Collectors and Tahasildars are the Claim Enquiry Officers. After the sanction by the Collector-cum-claim settlement o commissioner, the sanction order along with other requisites are to be forwarded to other National Insurance Company Ltd. for necessary payment .The amount will be disbursed through the claims Enquiry officers concerned.

Question No-5 How to apply by the Legal Heir of the victims and what documents is to be furnished at the time of apply.

Ans. The Legal Heir should apply in plain paper and in Form No. A (copy enclosed Tahasildars for compensation from Solatium funds with the following documents.

1. Death certificate.

2. Legal Heir certificate.

3. Copy of plain paper F.I,.R.

4. Copy of G.R case F.I.R

5. Copy of postmortem report.

6. Annual Income of the applicant.

7. Enquiry Report from the concerned Tahasildars.

Question No-1Do you want to purchase some handicraft materials for interior decoration?

Answer- Visit District Jail, Keonjhar where different types of colour bed sheets, Gamchha, Towels, Dusturs, Durry of different size, Assan Roll are being manufactured by the inmates and sold to public and supplied to different offices on demand. Apart from this Sofa sets, Racks, different type of mats, stools etc. are being manufactured by the inmates made out of Sabai grass. Similarly, Press Khali plate and cups are being manufactured and sold to the public.

Question No-2 Do you want to buy some fresh vegetables and fish?

Answer- Visit District Jail in which seasonal vegetables, Paddy etc. are being cultivated by the inmates and supplied to them for internal consumption as well as sold to the public. The Jail is having four numbers of Tanks in which every year Pisciculture is being done. During summer the fishes are caught and supplied to inmates for internal consumption and sold to public.

Answer- Yes, because generally antisocial elements are confined in the jail.

Question No-4 Do you have any ill feelings upon the prisoners?

Answer- Yes, people are being arrested by the police due to indulged in criminal activities.

Town Planning

Question No-1 What to do regarding approval of a plan?

Answer - The applicant has to purchase an application form from the office of SPA,Keonjhar, Joda,Barbil and Anandapur by depositing of Rs.5/-. He has to fill up the form and submit the same along with original plan, three copies of ammonia prints document in support of ownership of land . In case of conversion from agriculture to non agriculture purpose he has to get permission from the revenue authority. Scrutiny fee is also to be deposited by the applicant. There after site inspection by Amin is made and as per zoning regulation of the Master Plan and provision of Orissa Municipal Rules, the plan is scrutinized and approval accorded under section 31(3) or 33(i)of O.T.P.& I.T.Act.1956.

Question No-2 What is the main function of Town Planning?

Answer - The main activity is to prepare a Master Plan for the Urban area and its adjacent urbanisable area within the Master Plan limit for the plan horizon of 20 yards. The Master Plan earmarks different use Zones such as residential, commercial, industrial, agricultural and allied, transport and communication etc based on land use survey and after studying the economic base, population and other socio-cultural activities & potentiality of the Master Plan area.

Answer - (i) A Building Centre, a production-cum-Training Centre for cost effective materials is running under the O.I.C.              2.If the officer is appointing authority, he will forward your name with date of birth to the District Medical Board headed by Chief District Medical Officer or Regional Medical Board at Cuttack, Berhampur and Sambalpur.
 
            3.The respective chairman board or Convenor of the Board will intimate the date of your appearance before the Board.
 
            4.Reach and appear before the specified time and date.
           
            5.Report will be sent to the concern Officers that has given the requisition.
 
            6.If you are made invalid you will be allowed too retired with benefits as per rule
 
            7.No fee is charged for this.

Question No 2- Do you want a handicap certificate?

Answer- 1.Appear before Chief District Medical Officer with the prescribed forms available at Chief District Medical Officer office.

            2.On payment of Rs.5/-

            3.The form must contain the recent photograph exposing the defect of the body clearly.

              4. Enquire on which day the handicaps are examined.

             5.Chief District Medical Officer may sent you to a Specialist and gets yourself         examined.

             6.Appear before the Specialist and get yourself examined.

   7.Come back to Chief District Medical Officer with Specialist Report.

   8.After the formalities collect the certificate & go.

   9.No fee is charged.

Question No 3- Do you want a medical fitness certificate?

Answer-1. Be sure, why you want a Medical fitness certificate.

a. For joining for the first time in Government Service.

        b. Joining after leave on medical ground, joining of education institution or              any other.

          2. Please read the appointment letter reliable rules in a service code, the form for admission to educational institutional carefully.

            3.Meet the designated Doctor and get yourself examined and take your certificate.

    4. In the hospital, it is free.

           5. If you go to the residence of the protecting doctor then the doctor may charge   the fees.

Question No- 4 Do you want a birth certificate?

Answer-1. Please remembers whether you have registered the event or not.

    2. If you have registered the event then go to local register for a certificate.

           3. In rural area, the Medical Officer in charge of block PHC/CHC/UGPHC are the                   registrars of birth and death.

            4. In urban area, either the health officers or the executive officers are registered.

    5. Contact then as per Form. No.2 from the VS clerk.

    6. If you fail to provide the form contact the registrar.

    7. Fill up the form and deposit Rs.2/- with the register. Obtain a certificate.

Question No-5 Do you want a death certificate for your relation?

Answer-1.Please remember whether the death event has been registered or not.

            2.If registered, obtain an application form, fill it up and submit to the registrar with                 Rs.2/- for searching and certificate.

    3.Collect the certificate.

Question No-6 Did you forget to register the birth or death events?

Answer-1. As per law, the event should be registered within 21 days from, the date of occurrence.

    2. From 22 days to one month, by paying 1 Rupee fine it can be registered.

            3. From one month to one year, by paying Rs.3/-, it can be registered. But it will take sometime since the request will come from the registrar to the district register who after careful consideration will allow such entry and communicate the same to the register who after receiving the permission will entered the event in the register and issue certificate.

           4. If it is more than one year, then you have to go to an executive magistrate  court. There after you have to execute an affidavit and have to apply to the court with court fee , requesting the court to pass a order to the registrar to register the event in birth or death register as the case may be.

           5. After entry of the event into register, you can obtain a certificate by applying in a form available with the registrar with Rs.2/- searching and a certificate fees.

Question No-7 Did you not get good behavior from a doctor?

Answer-1. Talk to the Doctor politely.

    2. May be he is worried for some more serious cases.

    3. Place yourself in his position.

    4. A doctor is after all a human being.

            5. After considering all above if you feel that he is not behaving properly then complain to next higher authority in writing.

Question No-8 Did anybody harassed you in the hospital?

Answer- Complain to the superintendent of the hospital.

Question No-9 Do you want a cabin?

Answer-1. Talk to your treating physician.

            2. Get forward an application to Additional District Medical Officer or              Superintendent of hospital.

    3. Everyday at 11 O'clock, cabins are allotted.

            4. You should be present at that time and as per seniority cabin will be allotted.

            5. You get the cabin. If Not approach the Superintendent of the Hospital For District Head Quarters Hospital Chief District Medical Officer.

Question No-10 Do you want a food license?

Answer-Please collect the prescribed format available in the office of the licensing authority who is Additional District Medical Officer (Public Health) Keonjhar for whole of the district except the Keonjhargarh Municipality where the Health Officer is the licensing authority.

Apply with a challan form duly filled to the licensing authority

The licensing authority will pass the challan

Deposit the amount in the SBI

Submit the application form duly filled in with a copy of the challan received from the bank.

Wait for a week and collect the License . and continue the business

Question No-11 Is it mandatory?

Answer-Yes, it is mandatory , Running a food establishment without license is a criminal offence under the law of the land PFA Act 1956 and rules made there under.

Question No-12 Do you want to open a food establishment?

Answer-First of all after obtaining permission from Municipality where necessary Complete the construction work. Apply for food license with a challan filled in. Licensing authority will inspect the premises and if he feels the place is suitable for the purpose then he will pass the Challan for deposit of the fees. After deposit of fees, the license will be issued.
